Advance Care Planning with people with learning disabilities

A FREE learning event to be held in Torquay. Designed for health & social care professionals working at all levels.

Learning Objectives:

To understand the process of advance care planning and resources that have been developed for people with learning disabilities.

To recognise cues and facilitate conversations with people about their plans and hopes for the future.

To consider ways to enable people you work with to have choices at end of life.
Prerequisites:
Places are limited and restricted to two free places per workplace. Additional places can be booked and will incur a charge of £25 per person
